The Medal was instituted by the Soldaten-Siedlungs Verband Kurland (Soldiers Settlement Association of Courland) on the initiative of Karl, Freiherr von Manteuffel, chairman of the Association, and awarded from 1919 to 1923 to those who had fought voluntarily in the Baltic provinces of the former Russian Empire, Kurland (Courland)

The inscription ‘Naer Oostland willen wy ryden’ (We wish to ride to the East) is the first line of a mediaeval song of Flemish settlers in Germany.  The date 1184 refers to the arrival of the Augustinian monk Meinhard of Segeberg to preach the Gospel in Livonian lands. 

The Medal was designed by Karl, Freiherr von Manteuffel, engraved by the great Art Nouveau artist Emil Doepler (1855-1922) who also designed the coat of arms of the Weimar Republic and produced by Oertel of Berlin. 

The Medal is extremely rare.
